Oak Tree Academy must have on file, by the date of admission, a copy of the child’s completed immunization record. This record must be updated regularly as immunizations are received. Each child’s record must reflect an up-to-date status according to the Minimum State Vaccine Requirements for Texas Child-Care Facilities.

Documentation acceptable for immunization records must have been validated by a physician or other health-care professional with a signature or rubber stamp and include:
A) the child’s name and birth date
B) dates of immunizations (month/day/year)
C) the number of doses and vaccine type

These immunizations are required by the State of Texas:

1. Diphtheria, Tetanus Toxoid, and Pertussis Vaccine (DTP/DTaP/DT)
2. Hib
3. Inactivated Poliovirus
4. Measles, Mumps, Rubella
5. Varicella
6. Hepatitis B, Hepatitis A
7. Meningococcal
8. Rotovirus
9. Influenza
10. TB test (if required)

Immunization Exemptions

Exemptions for immunization requirements must meet criteria by the Texas Department of Health rules 25 TAC 97.62. Their web site is or you may obtain a copy from the local or state health department.

Liability Insurance

As required by law, Oak Tree Academy maintains liability insurance in the amount of $300,000.00 for each occurrence of negligence covering injury to a child while the child is in care at the center.
